Creating PDF Forms

I want to create PDFs with end user fillable forms in them. I know how to create PDFs from OpenOffice text documents, I know how to print to PDF from most other applications from my computer, and I know how to create forms using HTML. Is there any way to either combine that knowledge, or suggestions on a program I can use to do this project instead?

Hi there, Scribus is able to create pdf forms.

I recently started playing with Scribus for other projects. I will have to sit down and see what I can do with it for this too.
