If you have Openoffice.org installed then open WRITER, create the document you want and click on the PDF icon on the toolbar, or FILE > EXPORT AS PDF. You can mofify the document in writer and then resave in PDF as necessary. This is available on any application on the Openoffice.org suite. You can also open, modify and save MS Office documents.

And Scribus is capable of making proper pdf forms. There are plenty of tutorials out there, too
