    Which Dolphin addon context menu?
It is a plugin download link on the pacpl site's downloads page which redirects to:
    https://sourceforge.net/projects/pac...-%20Konqueror/
    Plugins
File Manager/Browser Plugins (Konqueror, Dolphin, Nautilus, Nemo, Thunar)
It is not provided in repos. Developer recommends installing pacpl from repos however plugins have to be downloaded separately from sourceforge.

That is the correct one I am using. Thank you for posting that.

Correct, it uses that script. It's from the developers official addons. See post #16 above.

          New update:
After I installed avconvf it actually worked converting audio-2-audio formats. But nothing else worked.
I restarted the desktop and tried using it again. I can get it to at least convert some files from video-2-audio but not all of them. After a lot of testing I have determined that the date (age) of a file is not a factor and the format (flv, mp4) is not a factor either. But many files will not convert. I just get no action, or I get a file that looks like it converted (mp3 or flac) but it's only 8k and contains nothing.
So on some level pacpl is functioning enough to get some of the intended purpose of it to convert files. But still weird issues. I think something is clearly missing or this is still the ffmpeg/avconvf bug? Not sure.

This tool is amazing when it works right because you can highlight a ton of files in a directory, right click and convert to your desired format. And you can convert multiple mixed formats at the same time. It's fast and easy. In KDE4 this was in the repos and installed perfectly with the Dolphin service menu access. Now it's not so easy in KDE5.

I also wanted to mention that I did do some testing in VLC to see if batch processing was possible, it did not used to be possible (or at least mixed formats were not) I cannot recall. But you can do that now in the Convert/Save menu choice. However you have to add all the files into the box. And for some reason when converting video to flac, you no longer get a total time on the file. So if a song is 2:06 long, VLC shows the time as 0:00 but will count down the playing time correctly (i.e -1:09). So weird since usually VLC is amazing. No problems converting mp3.

Which looks to be from 2014

though looking at the files, the helper script is the only part that might be 'old' but it looks fine. I don't know perl, but the general gist is not hard to follow what it is doing, and that looks fine, to my untrained eyes at least.

              .
The last time it installed and worked fine was in 16.04 LTS. After that it was not in the repos and I could not get it to function properly. I tried once in 18.04 but it just gave me nasty errors about being installed. I also do not know perl so... guessing and posting on the internets are my only options. O_o Sometimes Google is not your friend.

It sounds to me like the developer is the only one that can fix these issues in this case. Sorry that your app is not working as expected.
Hopefully you will find an alternative solution that meets your needs. Good to know about VLC. I personally do not have a need to manipulate audio and video files that often. I convert from ogg to wav or mp3 to wav sometimes on the rare occasion but thats about it. FFMPEG or VLC is all I need to process one file on the off chance. Let us know if you find something that fits your requirements, I'd be curious what it is. I am sure there is a tool out there besides pacpl that does this sort of thing in Linux.

Which places it within the context of KDE SC (KDE Plasma 4) and is (now) 8 years old! I'm not surprised that it isn't working, and supported by the OPs own experience:

I think this is a fruitless 'chase down a rabbit hole.'

I looked at the contents, just desktop file, an install script, and a perl helper script that basically calls simply pacpl. Nothing seems obviously broken, though at first glance the installer does put the desktop file in /usr/share/kde4/services. However, iirc this may still be a valid path in Plasma 5 (for legacy purposes), and is even still used - for the khelpcenter on my system (I have not checked if this is supposed to be like this. though)

                    Code:
                    claydoh@cubist:~$ kf5-config --path services
                    /home/claydoh/.local/share/kservices5/:/usr/share/kservices5/
However, it would be easy to test by moving the desktop file to ~/.local/share/kservices5/ or to /usr/share/kservices5/ and seeing if works differently. I doubt it, though.

pacpl is in the repos for 20.04. In fact, it's in the repos for 18.04, 20.04, 21.10, 22.04, and kinetic. But, it's in the Universe repo, and if that isn't activated/used, you won't 'find it in the repos'.
