Hi can anyone tell me how to fix this?
If you look at the following images, you will see that a lot of the icons and buttons on KDE specific apps. are missing, or just appear to be placeholders for these missing buttons.
I'm using the default KDE 'plastic' theme with tangerine icons.

Also while I have made my own Kmenu button, in the included images it is too small. I have however created the correct sizes for a replacement button 22x22 24x24 32x32, 48x48 and 64x64. It looks to me like it's using a 32x32 icon.
How do I make it bigger?
Can anyone please advise how to resolve this?
