Keyboard text overwrite doesn't work in KDE url box/filedialogs and Firefox etc

I first noticed in Konqueror trying to overwrite something on the url box, hitting insert key stays on insert mode, same in kde filedialogs and in Firefox url box. However the key works normally in Opera and text areas like in kedit etc.
