Long time since I talked about this problem.
Since 14.04 to 18.04 things have changed software wise, likely with NVIDIA Binary drivers. My hardware is the same.
I've been needing to update for a while now, the Nouveau drivers have been fine with the odd hard lock once a month. The lack of VDPAU hardware acceleration results in higher CPU usage and noisy fan syndrome, so now looking forward to quieter days.
So NVIDIA Binary drivers and older GeForce GT 220 hardware is capable of 2560x1440@60 via DVI-D.

PCI Express x16 Gen2
BenQ GW2760HS (HDMI-0) - 1920x1080
AOC Q3279WG5B (DVI-I-0) - 2560x1440
NVIDIA Driver Version: 340.107
X Org Server Vendor Version: 1.19.6 (11906000)
HDMI on these old cards with NVIDIA Binary are stuck on 1080p. If one needs to output higher than this Nouveau and xrandr new modes will likely be the only option.
DVI-D on these old cards with a good quality Dual Link DVI cable will do at least 1440p (in my case).
