No WiFi after waking from suspend

The WiFi on my laptop (Lenovo ThinkPad T410) won't wake up properly when I bring the computer out of suspend. The Networks widget claims that there is a connection but shows no data going in or out. The "ip a" command shows that wlp3s0 has acquired an IP address, or at least remembers the old one. Below is a list of things that sometimes fix the issue:
    • disconnecting and reconnecting to the WiFi network
    • disabling and re-enabling WiFi using the Networks widget
    • restarting networking from the command line "sudo systemctl restart NetworkManager"
    • suspending and waking the computer again

However, the only sure way to regain WiFi is to reboot the machine.

The WiFi has been kind of flaky ever since I installed 16.04 some months ago, but it has gotten somewhat worse after updates. I have all the latest updates installed. On 14.04 I didn't have any WiFi issues on this same machine.

I finally had some time to test this, so I did some clean reinstalls on a new drive. On Kubuntu 14.0.4 the WiFi resumed from suspend 10/10 times. Both Kubuntu 16.04.1 and Ubuntu 16.04.1 resumed like 2/10 times. So this doesn't seem to be a hardware issue and it also affects Ubuntu.
