So the uploader is Flash. Interesting. Using Chromium's element inspector, I can see that, indeed, the button will call a Flash method. Flash is written in Gtk+. So perhaps this is the issue -- Flash doesn't know how to use KDialog.
Would you please check what I asked about in post #12?
Thunderbird uses much of the same code base as Firefox, as both are written in XUL with some Gtk+. Without the extra Blue Shell integration work, Firefox uses Gtk+. The Blue Shell PPA for Firefox adds KDE integration. Blue Shell would need to do similar integration work for Thunderbird to get it to use KDialog.

Install the user-agent switcher for Chrome extension. Then go to your OWA URL. Now click the little page-with-glasses icon next to the URL bar:
Change the user-agent to Firefox | Windows Firefox 15. Voila, new UI.
